Assumption Health Services

Assumption Health Services strives to provide excellent care to our clients in the privacy of their own home.

Assumption Health Services are provided at both John Paul Apartments and Assumption Court.

We also offer a limited amount of these services at other facilities registered as a Housing with Services establishment. Currently we are offering these limited services at Springhaven / Pine West II apartments in Cold Spring.

Both facilities are licensed by the state of Minnesota for Housing with Services and all services are provided by our licensed Class F Home Care Department (Assumption Health Services)

Services available:

· 24 hour on site supervision by Registered Nurse trained staff.

· There is also a Registered Nurse on-call 24 hours per day 7 days per week.

· Shower/whirl pool assistance

· Dressing and grooming assistance

· Escorts to meals and activities

· Minimal assistance with privately cooked meals

· Assistance with toileting and incontinence based on need
· Transfer and ambulation assistance (Maximum physical assistance of one provided)

· Medication Management Packages ( set up, reminders, and/or administration

· Laundry / housekeeping

Other supportive services through the Assumption Community

· Social service support

· Congregate Meals at both facilities

· Out-patient therapies

· Secure building

· Volunteers
