What does a social worker do?

 

The social workers at Assumption Community help residents and families in a number of ways.  They serve as advocates to ensure resident rights and provide emotional support to residents and their families.  In addition, social workers can help by:

  • Working with families and medical providers to coordinate admissions to the nursing home,
  • Monitoring the psychosocial well being of residents, including their cognition and mood,
  • Helping families apply for Medical Assistance or other financial programs,
  • Assisting residents and families in creating Advanced Directives,
  • Coordinating discharges back to the community when a resident no longer requires skilled nursing care,
  • Offering support and resources for end-of-life care,
  • Arranging for family care conferences, and
  • Leading Resident Council meetings, Family Council meetings, and Support Groups.
