Transitional Care at Assumption Home

Transitional care serves those who have been discharged from the hospital but still require short-term rehabilitation and special care in order to make the transition from hospital to home.

Assumption Home provides  5 day  per week Physical, Occupational, and Speech Therapies, Ongoing Discharge Planning , along with other special care needs i.e. IV's, Wound Care, Clinical Monitoring.
