Assumption Home Clinical Team

We would like to take this opportunity to provide you with an introduction to some of the staff that makes Assumption Home the great place to live that it has become! Below you will find pictures along with a small biography about how they contribute to, and strive to provide the best for your loved one while a resident at Assumption Home.

 

 

heidiHeidi Hoppe, RN / Director of Cinical Operations

  Heidi started at Assumption Home over 5 years ago as a RN Clinical Coordinator. She was named to the head of our Home Care department last year, and now oversees all facility nursing operations as our Director of Clinical Operations. She contniues to pursue her education and is currently working towards completing her masters degree in nursing.

 

 

 

 


 

Registered Nurse Clinical Coordinators (RN/CC) are unit managers that are responsible for the day-to-day clinical cares of the residents. The RN/CC oversees each residents medical care and coordinates their care with the rest of the medical team including physicans, therapists, and dieticians. They are involved with care planning, goal setting, and the RAI process for Medicare.

Julie Pfannenstein, RN / Director of Clinical Reimbursement

  Julie has worked at Assumption Home for over 10 years. She comes to us with years of nursing knowledge, and is the crucial team member that oversees the RAI process and helps  residents and their families understand and navigate insurance and Medicare processes.





 

 

 


  

Kathy Utech, Certified Dietary Manager

 

 

  Kathy has worked at Assumption Home for over 25 years. She brings a long, collective background to her position, and has worked as a Dietary Aide, Baker, and Cook. Kathy has been in charge of the Dietary Department for over 5 years and works with the Clinical Team to provide comprehensive, well-balanced menus for the residents.
