Assumption Home
Assumption Home is an 82 bed, skilled nursing facility (SNF) located on the Assumption Campus in Cold Spring, Minnesota. Assumption Home is renowned for being a christian health care facility that strives to provide long term and rehabilitive care for older adults.
Assumption Home is a single story structure, conveniently located in the center of Cold Spring, and is bordered by numerous picturesque scenes including a garden, park, and the middle school playground.
Assumption Home offers the older adult and their families numerous services and aspects of medical care including several rounding onsite primary physicians, numerous consulting physicians including an audiologist and podiatrist, and a local pharmacy to provide medications. Assumption Home is also able to coordinate your loved one's care with other clinics and medical professions as need.

Why would my loved one need a nursing home (skilled nursing facility) or the services they provide?
Nobody likes to think of the day when they will need help to complete their own basic cares and activities of daily living. However, this is a moment that we all face in life. This year alone, about nine million men and women over the age of 65 will need some type of long term care. By 2020, 12 million older adults will need long term care. A study by the U.S. Department of Health and Human Servicessays that people who reach the age of 65 will likely have a 40% chance of entering a nursing home, or skilled nursing setting.
While Assumption Community offers numerous services in order to best equip yourself, or your loved on to stay within their home setting as long as possible, there may come a time when you require the services of the Assumption Home. We at Assumption Home are equiped to provide skilled nursing services, in-house rehabilitation (including Physical, Occupational, and Speech), and an in-house kitchen with meal services three times day. We have experience admitting people for numerous reasons including, but not limited to:
- Aftercare following both elected and unforseen surgeries.
- Chronic and/or acute disease progression that is no longer able to be managed outside a medical facility.
- Inability for you, or your loved one, to stay safely inside their home or apartment settings.
- Respite care for caregivers in the home.

Will Medicare or Medicaid pay for my stay at your SNF?
Historically Medicare will only pay for a restorative, or short-term basis stay at a nursing home. Medicare does not cover ongoing maintenance or custodial care. Medicaid is an option, however requires certain criteria to be met. If you have questions regarding Medicare and/or Medicaid payment or eligability, we ask you to contact our Social Services department, and/or Julie Pfannenstein, RN.
Who will be taking care of me or my loved one?
Assumption Home uses a group of health care providers called an Interdiscipnary Team (IDT) to provide an excellent continuium of care. Assumption Home's IDT group consists of Registered Nurses, Therapists, Social Workers and Dieticans to manage your care and promote health and wellness. Please take a few minutes to meet some of the Assumption Home staff that help to create a unique, and pleasant home for you or your family members!
